1. Right to participate in the community. Can you choose where you live in the same way as people without disabilities? Do you have access to all the support services and community facilities you need because of your disability? Do you have access to all the mobility aids, devices and other support you need? Are they affordable?

2. Freedom of expression and opinion. Are you able to access government information and publications, like information on the disability pension? Is government information provided in your language and in accessible format?

3. Right to health. Do you have access to the health care and health information you need? Do you have any problems accessing health or life insurance because of your disability? Do health care professionals understand your disability?

4. Right to education. Do you feel you have the same education opportunities as others? Does your school/ university or training college make adjustments for your disability (eg do you get course materials in an accessible format/ do you get access to an interpreter?)

5. Right to employment. Do you feel you have the same work and employment opportunities as others? Does your employer make adjustments for your disability (do you get flexible working conditions/ interpreters for staff meetings etc?) Do you receive equal pay for your work? Is your workplace supportive of people with disability?

6. Right to life and bodily integrity. Do you feel safe in your community? Do you experience violence or abuse because of your disability? Are you are forced to get medical treatment that you don't want?

7. Right to family and parenting. Do you have difficulties accessing Assisted Reproductive Technology such as IVF? Do you have adequate access to support services to assist you to be a parent? Have you had a child forcibly removed from your family because of your disability? Have had any experiences of sterilisation? Do you get appropriate support to pursue relationships, including sexual relationships?

8. Adequate standard of living. Is your pension adequate to support your needs? Do you have any difficulties in dealing with Centrelink because of your disability due to, for example, poorly trained staff or information not being accessible to you?

9. Right to equality. Do you feel that you are discriminated against because of your disability? Do you feel that the disability discrimination laws adequately help you when you have been treated badly because of your disability?

10. Accessing the legal system and equality before the law. Do you feel that you have the same access to lawyers and court as people without disability? Can you access bank loans and credit in the same way as people without disability? Can you control your property and finances the same as people without disabilty?

11. Right to live where and how you want. Do you have difficulty moving between states and countries because of your disability? Can you choose where you live and who you live with? Do you have adequate living conditions and is your housing accessible?

12. Mobility and Accessibility. Can you adequately access public and private transport? Can you adequately access public and private buildings? Can you adequately access mobility aids and appliances.

13. Right to culture and leisure. Do you have access to plays, television, movies, tourism and other cultural events in the same way as others? Are you able to participate in sporting events in every way that you feel you are able to? Is your culture or ethnic group supported by service providers?

14. Access to the political system and political involvement? Are you able to cast an electoral vote, including accessing polling stations and ballot papers? Is information produced by political parties in plain English and in accessible format?
